Lately, acquiring Medical Services for injured students has become increasingly challenging without prior written consent from parents/guardians. To ensure prompt emergency assistance, please review the following statement. Also, note that this form will remain valid throughout your child’s enrollment in Pryor Public Schools, and any medical expenses incurred will be the responsibility of parents/guardians or their insurance company.
To ensure proper emergency assistance, I authorize Pryor Public Schools and their designated personnel responsible for the named child to seek all necessary medical care. Moreover, I authorize any licensed physician or medical personnel to administer essential medical treatment.
